This form is required for Septic to Sewer Conversions within Montecito Sanitary District Boundaries.
If there is additional scope of work to the Septic to Sewer Conversion, please start a Property Improvement Application. If an ADU is part of the scope of work, please start an ADU Application.

Project Cover Page
- Include total fixture count.
- Include square footage per building.
Existing and Proposed Site Plans, including:
- Utilty Plans
  - Include sewer lateral(s) and sewer main(s).
  - Pump System Specifications.
- Grading/Topography Plans
- Demolition Plans

Include District Easements on ALL plans.
